بيماران سکته قلبی بايد در طي بستري در بيمارستان، از نظر بروز مجدد سكته قلبي ارزيابي شود ، چون احتمال بروز اين خطر زياد است. ممكن است براي بيمار تصميم گرفته شود كه پيوند عروق كرونر انجام شود، يا از روش هاي ديگر مانند استفاده از بالن يا فنر (استنت) براي باز كردن قسمت هاي تنگ شده شريان هاي عروق كرونر (آنژيو پلاستي) استفاده گردد. زماني كه شرايط بيمار تثبيت شد، اقدامات بيشتري براى بازگرداندن جريان خون كرونر صورت مي گيرد.
روش های آنژيو پلاستی عروق كرونر
آنژيو پلاستي عروق كرونر شامل سه روش است:

استفاده از بالن برای جلوگیری از سکته قلبی مجدد
در اين روش با استفاده از روش آنژيوگرافي با كاتتر وارد شريان كرونر بسته شده مي شوند و با عبور يك بالن كوچك از درون و بازكردن آن در محل شريان بسته شده، تنگي را باز مي كنند تا جريان خون برقرار شود . سپس هواي بالن را خالي كرده و كاتتر را از رگ خارج مي كنند.

جلوگیری از سکته قلبی مجدد با برداشتن پلاك ايجاد شده در رگ
در اين روش نيز با استفاده ازروش آنژيوگرافي، پلاك منطقه بسته شده شريان توسط يك وسيله باريك وارد شده به رگ، تراشيده مي شود.

جلوگیری از سکته قلبی مجدد با گذاشتن استنت
در اين روش نيز با استفاده از روش آنژيوگرافي با كاتتر وارد شريان كرونر بسته شده مي شوند و از انتهاي كاتتر يك سيم فنري شكل كوچك و ظريف توخالي را داخل محل شريان بسته شده قرار مي دهند تا منطقه مسدود شده رگ را باز كند اين فنر درهمان محل شريان باقي مي ماند. ممكن است در بعضي از افر اد مجراي شريان پس از مدتي مجددا تنگ و نهايتاً دوباره بسته شود. اين مسئله در افراد سيگاري و ديابتي شايع تر است.

آنژيو پلاستی و جراحی پيوند عروق كرونر (بای پس )
در اين روش قسمت هاي كوچكي از سياهرگ (معمولاً سياهرگ هاي ساق پا ) يا شريان داخل سينه را برمي دارند و با استفاده از اين رگ ها از شريان آئورت قلب شاخه اي فرعي به انتهاي مسدود عروق كرونر متصل مي كنند تا مسير فرعي جديدي بر اى عبور خون در قلب ايجاد كنند . در مواردي كه نتوان از آنژيو پلاستي استفاده كرد (به علت طولاني بودن و سفتي لخته يا نامناسب بودن محل انسداد) و انسداد مجدد رگ ها كه نتوان با آنژيوپلاستي مجدد آن را باز كرد از جراحي پيوند عروق كرونر استفاده مي شود . معمولا متوسط عمر باز ماندن رگ پيوند شده جديد 10 تا 13 سال بعد از جراحي مي باشد.
