فشارخون شریان ریوی، بیماری مزمن اما جدی‌ای است که به آن مختصرا «فشارخون ریوی» گفته می‌شود. این بیماری در تعریف عبارت است از فشارخون شریان ریوی بیشتر از ۲۵ میلی‌متر جیوه در حالت استراحت، فشار وج ریوی کمتر از ۱۵ میلی‌متر جیوه و برون‌ده قلبی طبیعی. شیوع و بروز فشارخون شریان ریوی حدود ۱۵ مورد در هر یک میلیون نفر و ۴/۲ مورد از هر یک میلیون نفر جمعیت در سال است.
داروهای مسبب فشارخون شریان ریوی عبارتند از آنورکسی ژنیک‌ها (مانند آمینورکس، فن‌فلورامین)، آمفتامین‌ها و داروهای سروتونرژیک (مهارکننده‌های انتخابی بازجذب سروتونین، پرگولاید).
علایم فشار خون شریان ریوی
تظاهرات بالینی فشارخون شریان ریوی عبارتند از تنگی‌نفس پیشرونده، ضعف، درد قفسه‌سینه و سنکوپ. ادم اندام تحتانی و محدودیت در انجام فعالیت‌های بدنی نیز در مراحل پیشرفته بیماری مشاهده می‌شوند. به‌طور معمول تشخیص بیماری پس از ۲ سال از شروع علایم (حدود ۳۷ سالگی) گذاشته می‌شود.
درمان فشار خون بالای ریوی
فشارخون شریان ریوی، بیماری مزمنی است که با مرگ‌و‌میر همراه است. این بیماری در نتیجه برهم خوردن تعادل میان مدیاتورهای متسع کننده و تنگ‌کننده عروق ایجاد می‌شود.
درمان‌های فشارخون شریان ریوی عبارتند از بلوک‌کننده‌های کانال کلسیم، آنتاگونیست‌های گیرنده اندوتلین، مهارکننده‌های فسفودی استراز و آنالوگ‌های پروستاسیکلین. ابتلا به فشارخون شریان ریوی متاثر از چند عامل است که در مجموع به افزایش مقاومت عروق ریوی در نتیجه تنگ شدن عروق، مسدود شدن، التهاب یا ترومبوز منتهی می‌شود.
اندوتلیال مختل باعث تولید گشادکننده‌های عروقی (مانند اکسیدنیتریک، پروستاسایکلین‌ها، پپتیدهای روده‌ای وازواکتیو) و موادی مانند ترومبوکسان A2 و اندوتلین می‌شود. این مواد در پارانشیم ریوی تغییرات دائم ایجاد می‌کنند. افزایش مقاومت عروق ریوی باعث هیپرتروفی و گشادی بطن راست و در مراحل بعدی، نارسایی بطن راست و مرگ می‌شود.
اپسومیت (Opsumit)، یک داروی جدید برای درمان بزرگسالان مبتلا به فشارخون بالای ریوی- که یک بیماری مزمن پیشرونده است و باعث مرگ یا نیاز به پیوند ریه می‌شود- را مورد تایید قرار داده است.
اپسومیت، به گروه دارویی‌ای متعلق است که بلوک‌کننده‌های گیرنده اندوتلین نامیده می‌شوند. این داروها با شل کردن شریان‌های ریوی، باعث کاهش فشارخون در ریه‌ها می‌شوند. اپسومیت مشابه سایر داروهای هم‌خانواده، در برگه اطلاعات دارویی خود حاوی هشدار در مورد ممنوعیت مصرف در زنان باردار است، بنابراین زنان مبتلا به فشارخون بالای ریوی شرکت‌کننده در این مطالعه بالینی قبل از شروع درمان با اپسومیت از لحاظ بارداری بررسی شدند و روش پیشگیری از بارداری مطمئنی برای آنها مورد استفاده قرار گرفت.
