علائم انفارکتوس حاد
علائم درد قفسه سینه شبیه به آنژین است ولى شدیدتر و پایدارتر (بیش از 30min) مى‌باشد و استراحت یا نیتروگلیسیرین بهبود کامل نمى‌یابد و اغلب با تهوع، عرق‌ریزش و احساس ترس (Apprehension) همراه است. با این حال ۲۵% از موارد MI از نظر بالینى خاموش مى‌باشد.
معاینهٔ بالینى انفارکتوس حاد
معاینه بالینى ممکن است رنگ‌پریدگی، عرق‌ریزش (Diaphoresis)، تاکى‌کاردی، S4 و ایمپالس قلبى دیس‌کینتیک (Dyskinetic) وجود داشته باشد. در صورت وجود CHF: رال، S3، اتساع وریدهاى ژوگولار در انفارکتوس بطن راست شایع است.
درمان انفارکتوس حاد
هدف، تخفیف درد، به حداقل رساندن وسعت بافت انفارکته و پیشگیرى ودرمان آریتمى‌ها و عوارض مکانیکى است. آسپیرین بایستى فوراً تجویز گردد (۳۲۵-۱۶۰ mg را در زمان مراجعه بجود و سپس ۳۲۵-۱۶۰ mg در روز به‌صورت خوراکى داده شود) براى MI داراى موج همراه با بالا رفتن قطعى درمان ترومبولیتیک زودهنگام با استرپتوکیناز، رتپلاز، یا فعال‌کنندۀ پلاسمینوژن بافتى (tPA) مى‌تواند اندازهٔ انفارکوس و مرگ و میر را کاهش دهد و اختلال عملکرد بطن چپ را محدود کند. در کاندیدهاى مناسب (اشکال – رویکرد به درمان ترومبولیتیک در بیماران مبتلا به MI حاد – و – تدابیر درمانى در بیماران مشکوک به بالا رفتن قطعهٔ ST.) بایستى ترومبولیز را در سریع‌ترین زمان ممکن (به‌طور مطلوب در 30min) در اتاق اورژانس یا واحد مراقبت کرونر (CCU) آغاز نمود.
